With changes in leadership, focused priorities and strong core values, PFH remains a "Good Family to Know!"Your role at PFH:As a Case Manager, you are responsible for facilitating child and family team meetings, and coordinating with guardians and other team members and coordinates the intake of new clients. You will:Demonstrate understanding of health service systems and assist program participants' in identifying, securing and sustaining a range of resources needed to facilitate independent healthy living.Serve as a member of a multidisciplinary team, supporting the development of the Comprehensive Treatment Plan based on individualized strengths, needs, abilities, and preferences of the person served; coordinates team input into the plan.Consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining goals as identified in the treatment plan; coordinates behavioral health referrals and ensures linkages.Provide direct services to children and families; coordinate family involvement in the service-delivery process.Provide individualized case management and activity of daily living services as needed.Evaluate services that are provided to ensure that necessary improvements are made to increase outcomes.Provide continuity of contact for the program participants; serve as a primary liaison to all team members.Monitor and assess program participants' functioning and progress; update team members with information relevant in supporting the treatment plan goals.Broker community resources and assist participants with linkage to family members and family support.Conduct appropriate assessments to aid in the development of the individualized treatment plan.Provides or arranges transportation to participants' community appointments as necessary.We think you'd be great for this role if you have:Evidence of at least two (2) years of responsible employment historyGraduate from an accredited college or university with Bachelor's degree in the Human Services fieldYou'll love being a part of our family because:Our associates are our most valuable asset!
