Job SummarySSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay and beyond.Key Responsibilities

• Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
• Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health.
• Utilize evidence-based practices to reduce readmissions and improve patient outcomes.
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
• Participate in quality improvement initiatives to enhance the patient experience and improve care delivery.Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of practice.
  • At least 3 years of acute care experience.
  • Strong communication and interpersonal skills.
